Crashes During the Past 12 Months
Have on hand the exact date of the crash, the names of known drivers, and the city and county of the crash.
Visit the branch of the Florida Highway Patrol (FHP) in the county of the accident. For a list of all FHP branch addresses, go online to flhsmv.gov/fhp/misc/CrashReport/Where.htm.
Explain to the FHP representative that you would like to request a Traffic Crash Report. She will provide you with paperwork. Bring a photo ID. You will be required to prove your identity and your relationship to the involved parties in the accident.
Crashes More Than 12 Months Ago
Fill out a Request for Report form, which can be downloaded as a PDF at flhsmv.gov/fhp/html/general/RequestForReport.pdf. Enter the date of the crash, location, and names of involved parties. Check off the box that identifies your role
as an involved party, or your relationship to one of the involved parties.
Take the Request for Report form to be notarized by a notary public, or signed certified law enforcement or correctional officer. Be sure to have a current photo ID with you.
Write a cover letter to the Department of Highway Safety & Motor Vehicles to accompany your Request for Report form. Type your letter to ensure that it's legible. Include your name, the exact date of the crash, the names of known drivers involved in the accident, and the city and county of the crash. Include your most recent contact information, such as address and phone number.
Enclose a check or money order for $10 made out to D.H.S.M.V. and a self-addressed stamped envelope.
Send your letter to the Department of Highway Safety & Motor Vehicles, Room B-310, MS-28, Tallahassee, FL 32399-0537. Keep a copy for your records.Source: ehow.com
