Planned FMLA Leave

According to Department of Labor regulations, you must notify your employer at least 30 days in advance for a planned event that qualifies for FMLA leave. This includes maternity leave, a planned medical procedure or caring for an ill or disabled parent, spouse or child. In addition to submitting an FMLA request form, your employer may require third-party certification, such as a form filled out by a health care professional describing a medical diagnosis and the amount of care needed to support the request. Although written request forms vary between employers, most require that you provide a reason and identify the start and anticipated end date. Because many employers require that you use accrued paid vacation time before going on FMLA unpaid leave, you may also need to include information about how much accrued time you’ve accumulated.
