Do you need to provide your birth certificate for a job or for another important reason but have just realized that your copy is missing? It happens frequently, and fortunately, you can get a new copy of the birth certificate using information found on the United States Government's Centers for Disease Control and Prevention website. Each state charges a different fee for birth certificate copies, and this fee must be paid at the time of your request.


Launch a Web browser, and then navigate to the Centers for Disease Control and Prevention's "Where to Write for Vital Records" page (see link in Resources).

Click on the name of the state in which you were born. There are a couple of exceptions to this: New York City and Washington D.C. have separate categories. If you are an American citizen who was born outside of the U.S. to an American citizen at the time of your birth, choose the "Foreign or High Seas Events," "American Samoa," "Northern Mariana Islands" or "Guam" option, selecting the one which most closely matches your


Click on the "Our Website" link on your state's information page, and then click "Get Birth Certificate." You will be routed to the state's online vital records site.

If an "Our Website" link does not appear, your birth state does not allow for online ordering of vital records. However, the specific information on how to request the birth certificate will appear on the state information page. This information includes telephone contact numbers, an address for the vital records office and directions for requesting the birth certificate by telephone or postal service.

Click "Start Your Order" to launch the request form, and then enter your full legal name, social security number and date of birth into the online forms when prompted.

Enter your place of birth county, mother’s maiden name and father’s name if it was listed on your original birth certificate when prompted.

Enter your payment information, and click "Confirm" when you are prompted. Information about turnaround time will appear at this time. Expect to receive your birth certificate via the United States Postal Service.
