Birth certificate information (i.e. full name, parents' names, date and place of birth)
In Person or By Mail
Find out if you meet the eligibility requirements for obtaining a birth certificate in San Antonio. You must be related to the person named on the birth certificate in one of the following ways: self, parent, legal guardian, child, sibling, step-parent, step-sibling, grandparent or spouse.

Prepare the appropriate fees for ordering a San Antonio birth certificate; it costs $23 for each copy. Make the check or money order payable to the City of San Antonio. If you don't know the birth year, additional fees may apply.
Bring the application, payment and your state-issued driver's license or I.D. card to the San Antonio Metropolitan Health District office between 7:45 a.m. and 5:00 p.m. CST
