To write a job self-evaluation without having an easy list of your successes is a much more difficult task than it needs to be. Keep a file on your computer desktop where you can jot down the jobs that you've completed, hopefully, ahead of deadline, and any rewards you've reaped (such as 'employee of the month, etc.'
Document your job strengths in your self-evaluation
To write a job self-evaluation honestly, you much mention both your job strengths and your job weaknesses, but start with your strengths! Highlight the items that you were really outstanding on - items such as 'consistently exceeded expectations and produced high-quality work ahead of deadline.' Or, if you saved your company money, don't be shy about documenting the dollar amount in your self-evaluation.
Document your weaknesses but put a good spin on them
Your boss will particularly appreciate your honesty by seeing your weaknesses documented. But, instead of write a self-evaluation statement that says 'my computer skills suck,' consider instead ''My Word skills are lagging so I'd like to take a learning course in intermediate Word.' In this way, you've shown your initiative with your interest in further education while noting your honest weakness.
Write realistic goals for the next year
Think of what your organization needs from you next year and write realistic goals to meet those needs. If you have professional affiliations and you really want to dazzle your boss in your self-eval, consider writing a goal of presenting at a professional meeting (such as a convention).
